If you are considering enrolling your child into our school please download the enrolment
form and return via email or or drop in and see us at 1 Reservoir Street, Gunnedah
between 8:00am and 3:30pm, Monday to Friday during school term.
An enrolment form must be completed for each child attending pre-school and for
every year that child attends. It will be the parents responsibility to inform us of any
change of address, phone number, place of employment and emergency contacts.
On enrolment you are asked to provide the following:
Immunisation record (Centrelink)
Documented proof of income if applying for fee relief (taxable income under $40,796 per annum)
Medical Action Plan (if needed)
Dept of Education consent form
Concession card (Health Care Card)
If your child has asthma, allergies or a serious medical condition, a medical action plan must be completed and signed by your doctor. These medical action plans need to be updated annually. If the Medical Action Plan Form is not completed, your child’s enrolment will not be accepted.
If your child is returning for the following year, please fill in the re-enrolment form.
